Hart Health and Wellness Agency has introduced new patient advocate and patient companion services. These services are designed to assist individuals who require help navigating the complexities of health care services and to offer companionship to individuals who are hospitalized.
Patient advocates can play a crucial role in managing communications between medical teams and patients and ensuring that patients' needs and preferences are addressed. They may also assist with understanding diagnoses, exploring treatment options, and coordinating care transitions. Our patient advocates have extensive healthcare and case management experience. They can attend health care appointments to assist with inquiry, literacy and clarification. Patient advocates provide information, education and care coordination services.
Patient companions will stay with patients to provide support and company during hospitalization. They will also gather information during the hospital stay and communicate updates to the patient's family and loved ones. The service aims to ensure that patients are not alone during their hospitalization and that their families are kept informed of their well-being. Our patient companions do not perform clinical services. Our role is to support and promote patient safety, effective communication, and mutual understanding among the patient, family regarding information shared during the hospital stay.
